REQUIREMENTS FOR REGISTRATION AS A DESIGNATED MEDICAL PRACTITIONER

 

 

 

Application letter

To be able to register as a designated medical practitioner, you need to be aware of the requirements set in Regulation 4 of the Diving Regulations, 2001.

You will need to forward the following documentation to the Department of Labour:

  • Underwater Medicine training certification (The Basic Underwater Medicine certificate)

  • Proof of current registration with the HPCSA

  • Certified copy of your ID document

  • Application letter (see proforma on the left)

All these documents must be forwarded to Ms Maphoto at the Department of Labour.  The postal address to send this to is:
